Eligibility & Referrals for CASSP Services

Who is Eligible to Receive CASSP Services?

  • Any child from birth to 21 years old

  • Any child receiving or in need of multiple services is eligible for assistance from CASSP

  • Any child at risk for out-of-home placement

  • Any child experiencing diminished function in their physical, cognitive, emotional, behavioral, and/or social functioning

  • Families experiencing agency confusion regarding leadership in complex cases

  • Children, families or providers who are experiencing communication difficulties

  • Treatment that does not appear consistent or coordinated

  • Frequent Inpatient hospitalizations

  • Children experiencing service plans that have not worked

  • ANY FAMILY OR CHILD IN LYCOMING-CLINTON COUNTY

Anyone connected with the child or adolescent including:

  • Family Member(s)

  • School official(s) & Faculty

  • Agencies

  • Providers

  • County Authorities

Please Note: Referrals are encouraged to be completed with the CASSP Coordinator, but may also be completed by an outside organization or family member.

All original paperwork must be submitted prior to a meeting being scheduled.

(NOTE: Release must be signed by a parent/guardian and/or the child if they are 14 years of age or older.)
