Who is Eligible to Receive CASSP Services?
Any child from birth to 21 years old
Any child receiving or in need of multiple services is eligible for assistance from CASSP
Any child at risk for out-of-home placement
Any child experiencing diminished function in their physical, cognitive, emotional, behavioral, and/or social functioning
Families experiencing agency confusion regarding leadership in complex cases
Children, families or providers who are experiencing communication difficulties
Treatment that does not appear consistent or coordinated
Frequent Inpatient hospitalizations
Children experiencing service plans that have not worked
ANY FAMILY OR CHILD IN LYCOMING-CLINTON COUNTY

Anyone connected with the child or adolescent including:

School official(s) & Faculty
Agencies
Providers
County Authorities

Please Note: Referrals are encouraged to be completed with the CASSP Coordinator, but may also be completed by an outside organization or family member.

All original paperwork must be submitted prior to a meeting being scheduled.
